It is hereby as follows:

| 1. The Maternity Nurse’s job title is that
of a Maternity Nurse. |
| 2. The Client agrees to employ the Maternity Nurse for
______ weeks from _______ at a salary of £_______
gross per week. |
| 3. The Client agrees to pay a retainer of ½ the
agreed salary from the above start date until commencement
of the position if the baby is late. |
| 4. The Client agrees to pay overtime for any days
off that are worked at a pro rata fee of £______
per day. |
| 5. The salary is to be paid in pound sterling on
a weekly basis. |
| 6. The Client shall provide free board, accommodation
and travel for the Maternity Nurse. |
| 7. Should the booking be overseas then the Client
should understand that they are responsible for the
cost of a return flight, and travel/medical insurance. |
| 8. Hours of duty shall be 24 hours a day 6 days
a week. |
| 9. The Maternity Nurse shall be given 1 full day
of 24 hours off per week. |
| 10. The Maternity Nurse shall be responsible for
the care of the new-born baby for the stated period. |
| 11. Should the booking be cancelled, or finish
prior to date agreed above, then the Maternity Nurse
shall be paid one weeks salary, as loss of expected
earnings. |
